We are currently seeking an LPN for 2nd shift.
Responsibilities:

Follow the nursing model to promote holistic care for residents
-
Direct the daily activities of the nurses, CNAs, and CMAs in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures

Participate in the admission, discharge and transfers of residents as required
-
Assess changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly
-
Transcribe physician orders to medical records and carry out orders as written
-
Prepare and administer medications per physicians’ orders and observe for adverse effects
-
Schedule follow up appointments for residents

Provide nursing leadership to nursing team members assigned to the unit
-
Provide direct care such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc
-
Perform rounds to ensure resident needs are being met
-
Provide direct patient care, including vital signs assessment, medication administration, and patient evaluations.

Complete medical documentation using EMR (Electronic Medical Records)

Support residents with activities of daily living and specialized care needs such as Alzheimer's, dementia, and memory care
-
Collaborate with other members of the interdisciplinary team to ensure residents’ needs are holistically met

Assist in evaluating work performance of nursing personnel and implement discipline according to operational policies and as directed

Inform appropriate departments of concerns or complaints and follow up as needed
-
Report any incidents or unusual occurrences to the DON and participate in the investigative process as needed
-
Participate in end-of-life care as required

Ensure that there is an adequate stock of medications, supplies, equipment and notify appropriate personnel of needs

Initiate, review and update care plans as required

Participate in the survey process when inspection surveys occur

Ensure that infection control principles are followed and assist in correcting deficiencies in practice
Requirements:
-
Current unrestricted Iowa LPN license
-
CPR certification
-
Schedule availability to work every third weekend and every other holiday

Excellent interpersonal skills, including customer service, patience, empathy, and compassion
-
Experience working in long term care a plus but not required
Job Details:

Schedule: 2nd shift: 2:00-10:00 pm including working every third weekend and every other holiday

LPN starting wage: $30.00 per hour; plus pay for related experience
-
Shift differential for 2nd and 3rd shift: $2.00 per hour

Incentive pay for picking up additional hours
